Each Pennsylvania county has a slightly different process for chaning one’s legal name. A transgender name change in Pennsylvania is normally a straight forward process. Transgender residents of Pennsylvania may download blank copies of the Court documents for several Pennsylvania counties below.

If your county is not listed below you should visit the Prothonotary’s Office at your county courthouse and request assistance.
